blocked on a mutex. Lance's series extends this to semaphores

Wentao Liang addresses a couple of minor flaws in nilfs2

- "scripts/gdb: Fixes related to lx_per_cpu()" from Illia Ostapyshyn
fixes a couple of issues in the gdb scripts

Coiby Xu addresses a usability problem with kdump.

Eliminate the limitation by

- redefining macro ERRSEQ_SHIFT
- defining a new macro ERRNO_MASK instead of MAX_ERRNO for errno mask.

There is no plan to change the value of MAX_ERRNO, but this makes the
implementation more generic and eliminates the BUILD_BUG_ON().

At the same time convert users in header and lib folder to use new
header. Though for time being include new header back to kernel.h to
